You do not have to be retired military or a resident of Air Force Village to benefit from short-term skilled nursing and rehab-to-home care at Air Force Village. We provide in-patient and outpatient services to those living on our campuses and the surrounding community. The Mission - A New Way to Provide Skilled Nursing Care Nestled on the Air Force Village I campus, located near Lackland AFB in San Antonio, short-term rehab-to-home services soon will be provided in Crockett and Bluebonnet Households, custom built spaces opening in early fall 2011. Thanks to their modern design, open living areas and therapy center, Crockett and Bluebonnet households, with private rooms, create the warmth of home and provide ease of recovery for our short stay guests. The Mission is designed to connect six households, each with 13-14 clients, formulating a neighborhood within the rich and vibrant community of Air Force Village.
Air Force Village will deliver innovative, transitional, short-term skilled nursing and rehabilitative care in a home environment for seniors leaving the hospital following surgery or illness. We can accommodate up to 28 seniors in our rehabilitation households and can accept Medicare and other co-insurances. For further information, call (210) 568-5257. When you are recovering from surgery, an injury or illness, home is where you want to be. But if you are not quite ready for home, The Mission short-term care and rehab-to-home services are the next best choice. Air Force Village II Health Care Center The Village II Health Care Center is conveniently located a few miles west of San Antonio on 660 acres of rolling countryside. Here you will find quality of life and quality of care - in a warm, healing environment that feels like home. We understand that every senior's situation is individual and our goal is to meet your needs as you recuperate in one of our spacious rooms. Our short-term skilled nursing, wellness and therapy programs are designed to maximize clients' functional independence and return them to their homes. AFV offers an ideal setting for clients who require more than home health care, but no longer need a hospital stay. Healthcare services at both locations include:
At both locations, your rehabilitation will be led by a team of experienced professionals, including physicians, nurse practitioners, nurses, dietitian, therapists and a social worker. Daily activities offer our residents the opportunity to enjoy the company of others, including a host of wonderful volunteers, and to engage in stimulating social, recreational and spiritual programs that encourage mental and physical wellbeing. |
